SCHOOL SPORTS PHYSICALS

All student athletes are required to have a current sports physical approved by our school physician on file in order to participate.

Sports physicals must be done on a state approved form. (see "Sports participation forms" tap for link to state physical form)

Approved physicals are valid for 365 days from the date of the exam.
In order to ensure that there is ample time for the school physician to review all private physicals prior to the start of the season the following due dates are:
DUE DATES FOR PRIVATE PHYSICALS.*
Fall sports season: July 15th
Winter sports season: October 1st
Spring sports season: February 1st
*ANY PHYSICALS HANDED IN AFTER THESE DATES MAY NOT BE CLEARED IN TIME FOR TRYOUTS.
All physicals must be approved by the school physician, as per N.J.A.C. 6A:16-2.2 which requires the school physician to provide written notification to the parent/legal guardian stating approval or disapproval of the student’s participation in athletics based on this physical evaluation. This evaluation and the notification letter become part of the student’s school health record. You may also check your childs physical status on the parent portal.
Private physicals must be handed in
to the school nurse at your home school.
DO NOT
GIVE THE PHYSICALS
TO THE COACH.
There is also a drop box accessible
at each school during normal school
hours, if the nurse is not available.
