 All private physicals must be completed on the state physical form and can be done by your family physician (medical home), as well as many local walk in clinics that offer specials for sport physicals.

 

We would like to encourage parents to hand in private physicals as soon as possible.  The state requires all sports physicals to be reviewed by the school physician. 

 

Due dates are there to allow ample time for our school physician to review the physical and expedite your child's participation.

Physicals are good for 365 days from the date of the exam and must be current on the 1st day of official practice.
