Attention Parents/Student Athletes:
1. Log into the parent portal. (Link below)
2. Choose the "SIGN UP" tab on the left.
3. Choose a team then review and acknowledge all of the forms.
(all forms must be acknowledged to continue to register for a team)
4. Click the SIGN UP tab for the team you wish to be considered for.
(you may choose multiple teams, by acknowledging permission to participate for each team)
NOTE: Your physical must be current on the first day of practice.

FAQ's for Online sign up and acknowledgment frequently asked questions:
1: What is the on-line sign up for?
A: Forms required (by the State of New Jersey, the NJSIAA and the Old Bridge Board of Education) for participation, in athletics, are available for parents to review on-line. It is quick, convenient and provides the opportunity to do so in a timely fashion.
2: When can I sign up for a team?
A: You may sign up for a team after the portal is updated for the current school year (usually late in July). Once you have acknowledged each document you may sign up for as many teams as you would like by clicking the permission to participate and acknowledgment for that team and “sign up”.
3: Are these the only documents I need to review and acknowledge?
A: No. The State of NJ requires a Health History update to be filled out prior to participation and coaches may have additional contracts they want signed, including an emergency card. This is TO BE HANDED IN AT THE BEGINNING OF THE SEASON to the coach.
4: Where is the Health History update?
A: It is available on line and coaches have copies http://www.state.nj.us/education/students/safety/health/records/HealthHistoryUpdate.pdf
5: When and where do I hand in the Health History update?
A: It is to be handed in to the coach at the start of the season.
6: Why am I having problems viewing the documents?

7: After I click “sign up”, why does it say “opt out”?
A: When you “sign up” you are putting your child’s information on a coach’s roster as a potential candidate for a team. If you no longer wish to be considered for that team and would like to remove your child’s information from that roster, you may chose “opt out”.
