Sports Participation Forms

The following forms are required for participation.

THE FORM BELOW IS THE ONLY FORM TO BE SUBMITED FOR APROVAL. 
ALL OTHERS SHOULD BE KEPT ON FILE BY YOUR HEALTHCARE PROVIDER.

Health History Update

 

The  Health History Update will be shared with student athletes through teams and submit electronically by the parents.

The Health History Update is only for new health information since your last physical.
Please do not hand in with physicals.

 

FAQ's Sports Physicals Frequently asked questions:

 

1. Why do I need a physical?

A: The state of NJ (New Jersey Department of Education 2014; Pursuant to P.L.2013, c.71) requires a physical for participation on a school athletics team.

 

2. How long is my physical good for?

A: 365 days from the date of the exam. YOU MAY CHECK THE STATUS OF YOUR CHILDS PHYSICAL ON THE PARENT PORTAL. (Look at the expiration date)

3. Do I need a new physical if it expires during the season?

A: No, a physical only needs to be current on the student-athletes first day of (official) practice, for that season. However, they will need a new physical if they wish to participate in the next sports season.

4. Where do I hand my physical in?

A: Physicals may be handed in to your school nurse or at Old Bridge High School. There is a drop box by door 1 by the information desk, at the main entrance.

5. When is my physical due?

A: Due dates for private physicals are:

Fall season July 15th

Winter season October 1st

Spring season February 1st

This allows a proper amount of time to process the enormous number of physicals that need to be reviewed and any subsequent corrections that need to be made.

 

Frequent reasons physicals are not cleared:

1. Preparticipation Health History Questions answered Yes, but not explained:

All YES answers must be explained.

2. Incomplete or missing information: i.e. 

Parent signature

Student signature 

Date on the health history 

Doctor signature and stamp

Vision not recorded by physician (for Left, Right and Both eyes)

3. Your physician must sign the cardiac module.

4. If your child has been under a physicians care for Orthopedic, Cardiac Condition or serious illness a medical clearance must be handed in or on file in your child's medical record with the school nurse.
